Community
Based Care

Our services span more than 30 miles across nine service locations in the Greater Houston area. Through a longstanding collaborative partnership with SEARCH Homeless Services, New Hope Housing, and Temenos CDC, we provide on-site care to residents within six permanent supportive housing sites across the city. Care teams consist of a Registered Nurse, Behavioral Health Clinician, and Community Health Worker delivering medical, mental health, and care coordination services.

Street Medicine

Weekly, a team including a Physician, RN Case Manager, Medical Assistant, and Community Health Worker provides medical care directly to individuals living unsheltered.

In partnership with The Harris Center for Mental Health & IDD, we also offer behavioral health and psychiatric services on the street, and collaborate with Coalition for the Homeless of Houston/Harris County to support coordinated care and housing connections. Services include assessment, vitals testing, medication evaluations, and linkage to care.

Jail In-Reach

HHH’s Jail In-Reach program, REACH: Reentry Engagement and Community Health, connects people experiencing homelessness with healthcare and supportive services during incarceration and as they transition back into the community.

Through a partnership with the Harris County Sheriff’s Office, our team works with individuals in the Harris County Jail to identify medical, behavioral health, substance use, housing, and other needs; coordinate connections to community resources; and help create a plan for continued care after release.

By building connections before release and continuing support in the community, REACH helps reduce gaps in care and connects individuals with healthcare, medications, benefits, housing resources, and other services that support a successful transition.
